Mobile Urgent Care Service (MUCS)

This service is intended for patients who have an immediate need for a home visit due to frailty or a tendency towards frailty. When such patients require assistance, a nurse practitioner will conduct a home visit.

This process follows telephone triage, which is either conducted by their regular GP or by a clinician from their own medical practice.

 

During the home visit, the nurse practitioner will assess the patient's requirements, collaborate with the patient on their ongoing treatment and care plan, and engage other members of the primary healthcare or broader healthcare team as needed.

The key distinction in this approach is the emphasis on delivering a prompt response earlier in the day. This early response facilitates quicker mobilisation of care and support, ultimately aiming to maintain patients' well-being and safety within their own environment whenever feasible.
