I completed my medical training in 2001 at the University of Birmingham and moved to Dorset in 2002 to complete my GP Training.  From February 2006 I worked as a GP in Purbeck for 9 years and I developed a particular interest in the management of frail and complex patients by working part time in community hospitals too. In April 2015 I started work for WECS as Lead GP and have enjoyed evolving this service over time to streamline and improve care. I love living in Dorset with my family and enjoy hiking, running and mountain biking in my spare time.

I undertook my medical training at University College London, qualifying in 2005. I completed my GP training in Dorchester and have been working as a GP in Purbeck and West Dorset since 2010. In that time I have worked in community hospitals as well as setting up a frailty service in the practices I have worked at. I have been working for WECS since January 2019 and enjoy being part of the team supporting patients in their own homes as well as in residential or nursing homes.

I completed medical school at Hull York Medical school in 2009 and completed my GP training in Dorset in 2014. Having previously worked for the WECS team I spent some time undertaking research into Dementia at the University of Exeter and I have returned back to the WECS ream to support the ageing well clinic and patients in their own homes. I also teach at both Bristol and Southampton medical schools. I enjoy the outdoors, I’m usually somewhere hiking, running or camping in my free time.

Frailty Advanced Clinical Practitioner

I have been a physiotherapist for thirty five years working across primary and secondary care settings. Having gained an MSc in exercise and health my interests are encouraging physical activity and well-being in relation to frailty management, sarcopenia and re-conditioning. Following completion of my training as an advanced clinical practitioner in an acute frailty unit and primary care I look to use my experience to facilitate primary and secondary care partnerships in the management of frailty and ageing well. I am particularly interested in focusing on utilising local resources to provide patient centred care in a timely fashion and enhance quality of life for older people. I enjoy walking my dog and spending time with my family.

I qualified as a Registered Nurse in 2006 in Lancashire and I have worked in the UK and abroad in a variety of settings. I have now settled in beautiful Dorset and have been part of the WECS team since February 2019. I feel we really can make a difference to those living in their own homes or in a care home. I have a special interest in patients who live with Dementia. In my spare time I enjoy weight lifting and dog walking.

I completed my OT training at Bournemouth University in 2014. Prior to the completion of my training I worked in the community for a number of years in different roles including a rehabilitation assistant role and district nurse assistant role. Over the years I have worked in different teams located across North Dorset, Purbeck, West Dorset and Somerset. Following the completion of my training I worked in an acute hospital as a Junior OT and returned to the community as a Senior OT in a couple of different integrated community rehabilitation teams across Dorset. Providing therapy assessments in patient’s own homes. Frailty, palliative care, admission avoidance and rehabilitation are things that I am extremely passionate about. I believe wherever possible, patients should be supported in their own home environment. I joined WECS in October 2021 as a Frailty Occupational Therapist.

In my spare time I am often found with my family walking, kayaking or paddle boarding along the South Coast.

I qualified as a Paramedic at Bournemouth University in 2009. I have worked as a Paramedic in Weymouth and Portland for South Western Ambulance Service for 12 years. In this time, I have gained interest and experience in primary care. I started working as a Bank Community Acute Response Practitioner for the MUCS team in Weymouth and Portland. I have since started working with the WECS team from November 2021.

I completed my nurse training in The Queen Alexandra’s Royal Naval Nursing Service in Plymouth in 1982. Since moving to Dorset in 1999 I have worked in community hospitals and as District Nurse Team Lead in Weymouth. I joined the WECS service in 2017 as a Frailty Nurse Practitioner. I have enjoyed being a part of developing this team, and we continue to develop.

I completed my Registered Nurse Training in 1987 and worked in various hospitals for 8 years. I then moved into General Practice and was a Practice Nurse for 19 years, with a specific interest in COPD and Asthma. I am an Independent and Supplementary Prescriber. I also particularly enjoy working with the Elderly and I joined the WECS service in June 2018 as a frailty practitioner, caring for the housebound patients. I enjoy walking in the Dorset countryside and I am an avid reader.

I completed my nurse training at the Royal United Hospital, Bath in 1998. Since then I have worked in a variety of settings including; large teaching hospitals, primary care and community services.  These varied roles have contributed to me developing a specialist interest in diabetes management, which led me to working, most recently, as a Community Diabetes Specialist Nurse where my interest in frailty began, ensuring people receive the right care in the right place. I have continued my post registration education and I am currently working towards an Advanced Clinical Practice qualification.  In my spare time I enjoy walking my dogs, paddleboarding and swimming.

I have worked with elderly patients for a little over 32 years, initially in the private sector but for most of this time I have worked in various roles within the NHS. I am fully dedicated to working within the elderly sector and I achieve great fulfilment from the support I am able to provide to our patients. The main focus of my life outside of work is my family. My three children are now all adults. I enjoy spending time with them but I especially enjoy the delightful times I have with my two-year old granddaughter. Apart from anything else, she keeps me smiling and keeps me fit! I was born in a small village in County Durham but have lived most of my life in Weymouth and sometimes in Portland.  As well as family time I enjoy all aspects of the Dorset countryside and love to get out and about with my granddaughter and her family dog. I also enjoy baking and to sharing the outputs with the family!

I’ve worked in Health & Social Care for around 18 years with a variety of roles from Domiciliary carer to Team Lead in a care home – Support Worker in Orthopaedic Trauma and now Frailty Assistant – Person centred care delivered to a high standard is something about which I am passionate.

I’ve been a very busy Mother to 4 wonderful children, 3 sons and a daughter who make me proud every day. In my spare time I’m a Landscape Photographer, avidly chasing sunrise, sunsets and atmospheric conditions, the results of which have seen me published, exhibited and prints sold. I have many goals to take me forward but probably top of the list is Astrophotography in South America!

MUCS (Mobile Urgent Care Service) Team

The Mobile Urgent Care Service (MUCS) is for people who urgently need a nurse to visit them at home because they are frail or getting weaker. Before the nurse comes, a regular doctor or another healthcare person will talk to the patient on the phone. The nurse will then visit the patient at home, figure out what they need, and make a plan for their ongoing care. If necessary, the nurse will also involve other members of the healthcare team.

The goal with MUCS is to respond quickly, especially earlier in the day. This helps get care and support to the patient faster, trying to keep them well and safe at home whenever possible.
